Please Note:

1. This registration form is for admission Purpose for Nursery(Balvatika-1) / Pre-Primary (KG).
2. Those Candidates short-listed for admission will be informed through school website i.e. (www.columbiaschool.in) / School Notice Board.
3. The relevant documents should be submitted by parents during the filling of admission forms.
4. Please fill up this form very carefully as Child’s Name, Date of Birth, Parent’s Name should be written correctly. Thereafter any kind of corrections will not be allowed under any circumstances.
5. Distance between residence and the school should be stated correctly. Moreover, in case of any incomplete / incorrect information the Registration form may be rejected even after the admission is done.
6. In if the child requires any medical aid / help during the school hours due to any reason those expenses will have to be borne by the parent. Only First Aid will be provided by the school.
7. Documents must be produced in original for verification and self attested copy to be submitted at the time of admission.

     I hereby certify that the above information provided by me/us is correct and I / we understand that if the information is found to be incorrect or false, the ward shall be automatically debarred from selection /admission process without any correspondence in this regard.
     I / we also understand that the application / registration / short listing does not guarantee admission to my ward. I / we accept the process of admission undertaken by the school and I / we will abide by the decision taken by the school authorities.
